With our goal of a seamless patient experience at Fairview clinics, and continual improvement, we are requesting feedback on your experience scheduling a general clinic appointment (during the day) through the centralized scheduling communications center for one of the following clinics: Fairview Andover, Fairview Blaine, Fairview Brooklyn Park, Fairview Columbia Heights, and Fairview Fridley.  Please give us feedback regarding the below questions and any comments or suggestions on your overall experience.  Thank you for your time.
